Our Support Coordinators take a personalised and collaborative approach. We take the time to understand each participant’s goals, strengths, and individual circumstances, then assist in coordinating supports that align with their needs. Our focus is on empowering participants to make informed choices, build capacity, and maximise the value of their NDIS funding.

Support Coordination includes but is not limited to:

Explaining your NDIS plan and funded supports in clear, practical terms.

Assisting to identify goals and priorities.

Connecting you with suitable and trusted service providers.

Coordinating multiple supports to work together effectively.

Supporting service agreements and provider communication.

Monitoring supports to ensure they meet your needs and expectations.

Assisting with problem-solving and resolving service issues.

Supporting plan reviews, reassessments, and changes in circumstances.
