Community Nursing
Gem Complete Health Services specialises in providing care to clients with a wide range of needs, in the comfort of their homes.
We liaise with Discharge Planners in hospitals and offer timely follow up in the community to help patients settle back with support to manage their new circumstances at home.

Special attention is given to the elderly who are living at home. We uphold the value of independence and empower them with support to ensure that they can live safely in their homes. We provide early detection of condition change, assessment and advise the relevant support needed.

We arrange visits from our Clinical Nurse Assessor who will come to you and formulate an appropriate person centred care plan for you. We will then provide you with a free quote and discuss it with you to tailor it to your needs. Our services will be reviewed and evaluated regularly to ensure we are delivering high quality care to our clients.
Support Coordination
Our Support Coordinators will concentrate on strengthening our participants’ capabilities to assume responsibilities and tasks associated with the management of their supports.

Accommodation / Tenancy
Our Support workers will support our participants in securing suitable accommodation by liaising with the relevant stakeholders and advocate for our participants in rental lease agreements.
Specialised Accommodation:

Specialist Disability Accommodation is provided for participants with very high support needs. The accommodation allows supports to assist with the delivery of services that cater for significant functional impairment.

Our services include:

Helping with or supervising daily personal tasks to develop the skills of the participant to live independently.

Participation support to enable our participant to engage in community, social and recreational activities.

Performing essential house or yard activities that the participant is not able to undertake.

Performing critical house cleaning activities that our participant is not able to conduct.

Preparation and delivery of meals to our participant who is unable to do this themselves.
Therapeutic Supports
Our Therapeutic Supports objectives are to provide our participant with functional development through adjustment, adaptation and building the capacity to participate in the broader community.
Gem Complete Health Services will utilise a collaborative therapy team model that will render professional services by providing a wide-range of rehabilitative services in our participant’s homes. Being led by our participant’s physician to help him or her to regain the maximum level of independence by engaging and increasing the capabilities of our participant in the daily living skills.

Assist Prod-Personal Care / Safety
This service is for when you choose to personally care for your disabled family member but need a bit of help.
We suggest, source and supply products that you will need daily but may not have the time to search and source. We will tailor the products to suit your needs or as requested. The products include daily products such as continence aids, wipes, and specialised utensils.
Our process is to step in, understand your needs, receive your requests and advise on the best
products. We will then deliver the products to your door step. You will not need to pay on the spot as we make our own regulated claim and receive payment
from the NDIA.
